Terms & Conditions of Therapy Payments are at the appointment by cash or cheque, card payments cannot be taken on the premises, and you will be given the choice to pay on a pay as you go basis or payment for block bookings. Cheques maybe posted beforehand to my practice address, a receipt will be issued with all payments.
I do offer home visits by arrangement if needed, it is important that your home is a suitable environment for us to work in, if you have small children who need your attention or pets that maybe a distraction to you then your home may not be suitable, feel free to give me a call for clarification. There is no extra charge if the destination is within a 10 mile radius of my practice address.
Appointments and Cancellations:
Should you have to re-arrange an appointment this must be done with at least 24 hours clear notice, if you have to cancel or re-arrange any one appointment more than twice, without good reason, the sessions will be cancelled. Future appointments can be made either in advance, in which case you are more likely to get the same day / time, or from one session to the next which may mean that your regular day / time slot may not available.
As I work alongside the medical profession I always recommend that the client consult with the GP, this is to ensure their physical and mental well-being of the client is paramount. Occasionally, if required, written medical permission to proceed may be requested.
Prospective clients will be expected to undertake an initial assessment before further therapy can take place. This will take place during your free consultation and documentation. During the initial consultation an intake form shall be completed along with an outline of any session plans. A copy of this will be made available to you. Following each further appointment contact session notes will be made if needed these notes may be sent to your GP with your permission.
All personal documents and notes will remain confidential, unless for some legal reason I am required to release them to an appropriate authority, in this case I will inform you of the request. They will be kept under lock and key in accordance with the Data Protection Act 1998, and destroyed at the appropriate time as set down by the Act.
Should you arrive for a session whilst under the influence of alcohol or drugs, or you become abusive during a session then the session will be cancelled immediately and any money paid or owing will become due in full.
Potential clients are welcome to bring someone with you for your consultation. Having a third person present during therapy sessions is not advisable and is discouraged, unless the client is a minor. You are welcome to have someone drop you off or pick you up at the designated times as we do a have waiting room.
Termination of Therapy:
Sessions can be terminated by either party, at any point in time with at least 24 hours’ notice. Should the termination be made by the therapist, then you will be told why it has been terminated and what, if any, alternative treatments may be available to you in the local area.
I am accredited registered and licensed to practice as a Clinical hypnotherapist with the General Hypnotherapy Standards Council. I am also qualified and registered to teach Clinical Hypnotherapy. I hold all public and professional Liability Insurances with Holistic Insurance Brokers. My certificates of authentication are available for viewing upon request.
Finally, I realise seeking help can be difficult for people at time I am committed to doing all I can to ensure that your experience of therapy is going to be a positive one.
